    You will need:

    1. Properly completed application for a rebuilt title in the name of the rebuilder.
    2. Salvage affirmation identifying all replaced essential parts.
    3. Original invoices for all replaced parts.

    5. Valid driver’s license of the salvage vehicle driver.

    The salvage vehicle must be fully rebuilt and in good operating condition. The exterior, engine, and transmission areas must be thoroughly cleaned.
